This funding opportunity supports research to identify which parts of primary care most improve patient outcomes. Focus areas include care coordination, seeing the same provider over time (continuity), comprehensive and person-centered/whole-person care, and the quality of clinician–patient relationships and trust. Projects should study care delivered in primary care settings and address clinical areas unique to primary care.
